Position Summary: 

Under the direction of the Sr. Pharmacist Supervisor, incumbents rotate through the different areas of the Pharmacy learning: pharmaceutical patient care, administrative functions, drug information, drug dispensing, and staff coverage. The residency starts and ends the last week of June.

 

Responsibilities:   

Accurately reads and interprets physician orders

Accurately verifies physician orders in TDS

Appropriately and accurately prepares and distributes drugs to patients. compounds and dispenses extemporaneous preparations. dispenses controlled substances

Appropriately prepares parenteral products using aseptic technique

Appropriately reviews patient charts following lab values and other objective data, obtaining info as to allergies, concurrent medical problems, and medications related to the patient

Assesses, identifies, and addresses, on an ongoing basis, the special needs of all ages of patients when providing patient care

Assures optimal formulary compliance

Collaborates with other team members in providing correct drug and dosage form for age group

Demonstrates advanced knowledge of adverse drug reactions, causes, mechanisms of action and related clinical treatments

Demonstrates advanced knowledge of basic clinical pharmacology and pharmacokinetics

Demonstrates advanced knowledge of drugs, drug therapy and clinical therapeutics

Effectively performs assigned administrative functions (i.e., med error reports, ADR reports, special projects, committee recording secretary, etc).

Effectively performs assigned drug information reports (i.e., monographs, MUEs, NFDR reports)

Effectively uses all applicable computer systems

Implements appropriate age related pharmaceutical therapy

Insures all orders are delivered appropriately and in a timely manner to the patient unit dose cassettes, unit refrigerators, or other appropriate locations in the hospital

Participates in committee meetings and programs to promote patient care, provider understanding and optimal drug therapy

Performs and documents drug therapy interventions

Provides accurate drug information upon request in a clear and concise manner, ensuring highest level of performance with regard to current information on selections and use of DI resources.

Provides appropriate direction to technicians regarding daily work-flow and operations

Provides drug information as needed, checks for understanding.

Screens drug appropriateness for age and physical systems tolerance

Successfully completes the technical-distributive and order verification in Sterile Products

When required, collaborates with nursing to evaluate the effectiveness of therapy considering the age of patient

 

Required Qualifications:

Current BLS or ACLS upon hire

Must be eligible for, and obtain, CA Pharmacist license within four months of residency employment
