Position Summary: 

The incumbent provides strategic direction, leadership, planning, organizational and general management for all aspects of the Case Management program which includes Case management, Transitional Care Management, Social Work, Utilization Review, Denial Management and Clinical Documentation Improvement. Incumbent is responsible for development, implementation and ongoing success of department and related organizational objectives and goals.


Required Qualifications:

Ability to establish and maintain effective working relationships across the HealthSystem

Ability to maintain a work pace appropriate to the workload

Bachelor's degree in Nursing or Social Work

Demonstrated ability to create high-performing, interdisciplinary team

Demonstrated ability to lead change and effectively manage conflict

Excellent analytic skills

Excellent working knowledge of desktop computer productivity tools

Excellent written and verbal communication skills in English

Knowledge of CMS, Joint Commission, CADP, and other regulatory and compliance requirements

Minimum of 3 years management experience at the Director level in a relevant health care setting in areas relevant to Case Management

Minimum of 5 years of case management and/or social work experience in a health care setting

Must be a graduate of an accredited school of Nursing or Social Work, currently licensed in California or eligible for licensure

Must demonstrate extraordinary levels of customer service and transmit these behaviors to the entire care management team

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties

Outstanding interpersonal skills; ability to interact effectively at all levels of the organization from senior management to bedside staff


Preferred Qualifications:

CDI knowledge and experience

Knowledge of University and medical center organizations, policies, procedures and forms

Knowledge of patient safety strategies

Knowledge of quality improvement methodologies

MS, MBA or Masters in Healthcare Management or related field
